It manifests as subcutaneous nodules or thickened cords beneath the skin of the palm, often resembling fibrous bands. These cords progressively contract, pulling the fingers\u2014most commonly the ring and little fingers\u2014into flexion, limiting the ability to fully extend the hand.<\/p>\n<p style=\"text-align: justify;\">This condition is more frequently observed in patients with diabetes mellitus, smokers, individuals with hypercholesterolemia, those with a history of alcohol abuse, and patients with HIV\/AIDS, there is also a significant hereditary and genetic predisposition associated with the disease.<\/p>\n<p style=\"text-align: justify;\">Treatment Surgical. In recent years, minimally invasive techniques such as percutaneous needle aponeurotomy (PNA) have gained popularity. In cases presenting only with palpable nodules and no contracture, intralesional corticosteroid injections may be administered. This approach can reduce and soften the fibrotic tissue in approximately 50% of patients.  <\/p>\n<p style=\"text-align: justify;\">When a fibrous cord is present along with digital contracture, percutaneous needle fasciotomy (PNA) is performed under local anesthesia, no skin incision and eliminating the need for hospital stay. Postoperatively, the patient is typically required to wear a custom splint to maintain finger extension. <\/p>\n<p style=\"text-align: justify;\">However, recurrence rates following PNA are approximately three times higher at the 5-year mark compared to conventional open surgical fasciectomy. Despite this, many patients opt for PNA to avoid the invasiveness and potential complications of open surgery. In the event of recurrence, surgical treatment remains a viable option. In addition, enzymatic fasciotomy using collagenase clostridium histolyticum injections is being employed as an alternative approach.
